Position Summary The Provider Relationship Manager is responsible for developing relationships with physicians and office staff that are engaged in chronic condition health management. The primary focus of the role is to serve as a liaison between Healthmap Solutions and physician practices, providing detailed program training, provider onboarding, introducing pay-for-performance (P4P) programs, and the overall general account management of the practice. The position will be market-based and will focus on large practices. Responsibilities Monitors and manages key deliverables of the physician office implementation and onboarding processes Builds strong cross-functional relationships with internal departments and physician partners and practice staff Introduced kidney health program to office staff and physicians Ensures service level agreements are met or exceeded Establish and manage performance incentives Evaluate and manage practices financial and clinical performance Establish and manage the local high-performance network of providers (HPN) Maintain high engagement and performance rates Support Quality Practice Advisors (QPA) initiatives Engage with key hospital systems Ensures timely and successful delivery of reports to internal and external stakeholders Maintains and documents all applicable practice and patient updates, follow up communications, and applicable dashboard information Establishes all critical workflow processes for each assigned practice Perform other duties/tasks as assigned Qualifications Requirements Bachelor's degree in healthcare/business/communications or related field is required 3 years of experience in communication, managed care, physician sales, or account management is required Relationship-focused - ability to develop proactive partnerships with physicians, office staff, and stakeholders Ability to problem-solve and execute initiatives Needs superb verbal and written communication skills Must possess a personal presence this is characterized by a sense of honesty, integrity, authenticity, confidence, and caring with the ability to portray the HealthMap solutions mission, vision, and goals Must possess the ability to close onboardings Need financial acumen and ability to perform basic financial analysis Need to be tech-savvy to work with businesses Business Intelligence tools Need to be highly motivated and goal-oriented. Able to work in a fast-paced environment Per Healthmap Solutions HR Security Policy, this position is considered HY (high risk for PHI and may require additional training/screening) Position requires travel within a geographic footprint Proof of full covid-19 vaccination Skills Excellent verbal, written communication, and presentation skills Ability to manage multiple priorities Must be proficient in Microsoft Office:

Salary Range:
$80K
Minimum Qualification
Communications & PREstimated Salary: $20 to $28 per hour based on qualifications.
